Commerce Act (E-Signature Act). Please read the following Consent and Authorization disclosure
and indicate your acceptance as instructed. We recommend that you print a copy of this disclosure
and maintain a copy for your records.

notices and disclosures in connection with your loan application by means of electronic delivery.
Your consent to receive electronic communications includes, but is not limited to:
Requesting Paper Copies. You understand and agree that you have a right to receive any required
notices or disclosures in paper form as well as your right to withdraw your consent to have them

your e-mail address by contacting us at (888) 880-5663.
How to Withdraw Consent. You understand and agree that you may withdraw consent by

will be considered “in writing”. You should print or download for your records a copy of your
notices or disclosures, your initial authorization to receive notices or disclosures, and any changes
to that authorization.
Federal Law. You acknowledge and agree that your consent to electronic notices or disclosures is
being provided in connection with a transaction affecting interstate commerce that is subject to the
federal Electronic Signatures in Global and National Commerce Act, and that you and we both
intend that the Act apply to the fullest extent possible to validate our ability to conduct business
with you by electronic means.

authorize Level One to deliver notices or disclosures to you electronically.
You understand and agree that Level One retains the right, to the extent permitted by law, to
amend this agreement in the future by providing you with written notice of such changes to your
last-known e-mail address. In the future, should Level One, by law or regulation, be permitted
to deliver any additional notices or disclosures to which you are entitled besides those specifically
listed herein, you hereby agree to receive such notices or disclosures in electronic

IMPORTANT INFORMATION ABOUT PROCEDURES FOR OPENING A NEW ACCOUNT OR FOR A REQUEST FOR CREDIT
To help the government fight the funding of terrorism and money laundering activities, federal law requires all financial institutions to obtain, verify, and record information that identifies each individual or business that opens an account or requests credit.
What this means for individuals: When an individual opens an account, or requests credit, we will ask for their name, address, date of birth, Social Security Number and other information that allows us to identify them. We may also ask to see their driver's license or other identifying documents.
What this means for businesses: When a business opens an account or requests credit, we will ask for the business name, business address, Employer Identification Number, and other information that allows us to identify the business and signatories. We may also ask to see other identifying documents showing existence of the business. USA PATRIOT ACT, Section 326
